Are you living with a chronic health condition?
Do you want to spend fewer days in the hospital and less time with doctors?
Join the Area Agency on Aging’s Chronic Disease Self-Management Program for a FREE six-week workshop to help you better manage your pain, health, frustration, fatigue, and feelings of isolation.

If you age 60+ or have a disability, and are living with any of the following conditions, this workshop can help you learn to manage your condition:
- Lung Disease
- High Blood Pressure
- Heart Disease
- Chronic Pain
- …or any other chronic condition.
This free six-week workshop will meet on Tuesdays at from Jan. 7 – Feb. 10 at the Verandas of Punta Gorda. The workshop starts at 12:30 p.m. and lasts for 2 1/2 hours.
