Are you living with a chronic health condition?
Do you want to spend fewer days in the hospital and less time with doctors?
Join the Area Agency on Aging’s Chronic Disease Self-Management Program for a FREEsix-week workshop to help you better manage your pain, health, frustration, fatigue, and feelings of isolation.
Space is limited and registration is required for this six-week program. To register, see below.
If you age 60+ or have a disability, and are living with any of the following conditions, this workshop can help you learn to manage your condition:
Arthritis
Asthma
Diabetes
Lung Disease
High Blood Pressure
Depression
Obesity
Heart Disease
Stroke
Osteoporosis
Chronic Pain
…or any other chronic condition.
This free six-week workshop will meet on Tuesdays at from Jan. 7 – Feb. 10 at the Verandas of Punta Gorda. The workshop starts at 12:30 p.m. and lasts for 2 1/2 hours.
The Chronic Disease Self Management Program is an evidence-based program developed by Stanford University. Funding for these free workshops is provided by a grant for empowering older adults and adults with disabilities through the U.S. Administration for Community Living and the Florida Department of Elder Affairs
