Intent to Request Direct Service Waivers
Area Agency on Aging for Southwest Florida (AAASWFL) will conduct a Public Hearing for its application for Direct Service Waivers from the Florida Department of Elder Affairs on Tuesday, August 6, 2019 from 10:00 a.m. until 11:00 a.m. at 15201 N. Cleveland Ave, Suite 1100, North Fort Myers FL 33903.
AAASWFL will request Direct Service Waivers to provide evidence-based health and wellness workshops under the Older Americans Act Title III-D (Disease Prevention and Health Promotion Services).
These workshops include:
A Matter of Balance
Arthritis Foundation Exercise Program
Powerful Tools for Caregivers
Stay Active and Independent for Life
Tai Chi for Arthritis (for Falls Prevention)
Tai Chi: Moving for Better Balance
Walk With Ease
The public is encouraged to attend and provide comments concerning the above services.
AAASWFL connects adults 60 and over and adults with disabilities to resources and assistance for living safely with Independence and dignity in Charlotte, Collier, DeSoto, Hendry, Glades, Lee and Sarasota Counties.
Any person requiring special accommodations to participate in this hearing is asked to advise AAASWFL at least five days before the hearing by calling 239-652-6900.
