Tailored Caregiver Assessment & Referral (TCARE)
Tailored Caregiver Assessment & Referral (TCARE) provides ongoing support to assist caregivers with navigating local resources and developing plans to deal with the unforeseen emotional, physical, and financial challenges of caregiving. Caregivers work one-on-one with a TCARE Specialist to pinpoint caregiving needs and goals and to create a plan of action to address them. Savvy Caregiver is a six-session, evidence-based series for caregivers of a person with Alzheimer’s or dementia. Caregivers engage with Savvy Caregiver concepts to make the most of their time with their person who is living with dementia.
