Join the Area Agency on Aging for SWFL to become certified as a workshop leader for the Stanford University-designed Chronic Disease Self-Management Program! (CDSMP)
The CDSMP helps people with chronic conditions to manage their own health, stay active, and take charge of their lives. Learn more about the CDSMP here.
This four-day training program is FREE with a commitment to lead CDSMP workshops on behalf of the Area Agency on Aging. You must attend all four days to training to qualify to receive certification.
Training is held on two consecutive Thurs/Fri: April 16-17 and April 23-24
The training location will be announced in the near future.

Funding for the Chronic Disease Self-Management Program is provided by a grant from the US Administration on Community Living through the Florida Department of Elder Affairs.
